Managing a long term health condition is challenging. People need to remember to take their medicines, visit their doctors, and talk to their healthcare team regularly. This is a challenge because people have to deal with many different tools and systems that don't connect with each other or doesn't have good usability. This often leads to forgetting to take medications and missed doctor visits. With more people dealing with ongoing health issues, we need one simple tool that makes it easier to take care of their health.
Most people with chronic health conditions rely on basic methods to manage their health like writing on calendars, setting phone alarms, or using sticky notes. These simple tools often don't work well enough because individuals forget to take their medicine, run out of medications, and miss their doctor appointments. They also have trouble reaching their doctors when needed or accessing their health records. Many are worried about keeping their health information private, so they avoid using digital health tools. This is why we need a health management app that's both secure and simple to use, but still does everything people need it to do.
Therefore, ChroniCare was created, which is a health app that helps people manage their medications with notifications, doctor appointments, and health records easily. The app is designed to be simple and personal, so anyone can use it, regardless of their age or experience with technology.
The app has a simple and easy to use home screen that shows you everything important on one screen such as when to take medicines, upcoming doctor visits, and important reminders. It sends helpful medication reminders, helps schedule doctor appointments, and lets you message your healthcare team directly when you need to refill prescriptions.
Because everyone has different needs, ChroniCare offers the choice of reminders through phone notifications, text messages, or emails, it offers tools to track your health progress and get helpful tips and it also has strong security to keep your health information private and safe though a sign in feature.
By making the app easy to use, ChroniCare helps make health management part of your daily routine. This means fewer missed medications, better communication with doctors, and better health overall.
In this case study, I will show how I built ChroniCare. I surveyed real users, created test versions, and made improvements based on feedback. The main goal was to create an app that truly helps people take better care of their health.
I created a survey to learn how people manage their health conditions. I wanted to find out how they keep track of their medications and doctor appointments. The survey helped me understand what problems people are dealing with, as well as how they prefer to manage their health, and how comfortable they are using health apps on their phones.
To create an online survey, I used Google Forms so that it could reach many people. also, before starting the survey, I made sure to notify participants what it was for and that their answers would be kept private. Here's what was described at the start of the questionnaire:
“The purpose of this survey is to design a health companion app that simplifies medication management and appointment scheduling for individuals with chronic conditions. The estimated time for this survey is 3–5 minutes. This is an anonymous survey, and your answers will remain private.”
To categorize responses effectively, the survey started with demographic questions, followed by questions related to medication tracking habits, appointment scheduling methods, notification preferences, privacy concerns, and overall comfort with mobile health apps.
